Transform's friend referral programme

Transform offers a 'Refer-a-friend' programme as a way of saying thank you to previous patients who have recommended us to their friends and family.

How to qualify

You must have had surgery or a non surgical treatment with us and you friend must have had their surgery or non surgical treatment within the last 12 months.

What are the rewards?

If your friend has had a surgical procedure with Transform, (Breast enlargement, liposuction, nose correction etc.) you will receive a cheque for £100.

If your friend has a non surgical treatment with Transform, (Line and wrinkle smoothing injections, dermal fillers, face peels etc.), you will receive a cheque for £25.

How to apply

Download our 'Recommend a Friend' Form and fill it out with your details and those of your friend whom you are referring. Send your form to your patient Care Co-ordinator at your local Transform clinic.

As soon as we receive the approved slip it will be processed and you will receive your payment.
 
*NB - The referred friend must have paid and taken their surgery or treatment before any payment is made.

*Please allow up to 4 weeks for your payment to be processed.
