This is the most common technique used worldwide and can be performed in all types of chests. It tends to have the lower risk of post-operative complications and can most of the time achieve a flat chest. Free nipple grafts can be positioned anywhere on the new chest. No nipple reconstruction is something that can be accommodated for those individuals that either do not want nipples on their chest or aim to have 3D nipple tattoos at a later stage.
This technique is good for patients with a medium sized chest who specifically want to try and keep the sensitivity of their nipples. It is an alternative to nipple grafting, and the scars are placed at the same place as with the free nipple grafts technique. The dermal flap is the skin from the bottom area of the chest, which has its top layer removed, so that the nipple can be placed inside the chest area and re-positioned in a more male-like position, still connected to its blood supply.
This is best for patients with a small chest, who have little or no breast skin droopiness (ptosis). A small amount of skin is removed to try and stretch the skin, and a scar is placed all around the circular areola. The areola can be made significantly smaller if desired. A permanent Gortex or PDS stitch is placed around the new sized
areola to try and stop it from stretching again post-operatively.
