The MHRA has today issued an alert notice concerning PIP implants used before January 2001.
Updated information from their French counterparts (AFSAPPS) suggest there is no guarantee implants manufactured prior to 2001 contained an approved filler gel. Their statement suggests 7,000 women may be affected. Their full statement can be read here http://www.mhra.gov.uk/NewsCentre/CON146714
What this means
Transform is disappointed on behalf of our patients that this further confirms both UK and European authorities have failed to effectively regulate the manufacture of PIP implants for a period in excess of 10 years.
The announcement itself falls short of being conclusive. In effect it means no party in the UK or France knows whether the pre-2001 gel material was approved or not.
While our immediate concern is for the group of patients potentially affected, today's announcement brings practical challenges for all cosmetic surgery providers. Transform are legally required to retain patient records for 7 years. In practice we retain them for 10 years. This means retrieving records for pre-2001 patients will be challenging - and in some cases impossible.
What you should do if you had your procedure before 2001
Should you have experienced no adverse symptoms to date, it is probable you have no cause for concern.
However, for your immediate reassurance:
- our use of PIP implants prior to 2001 is extremely limited
- the MHRA has said there is no link to cancer
- the MHRA and Department of Health has maintained throughout there is no safety issue relating to the gel material (approved or otherwise)

It should be noted that consensus amongst experts is any implant has a typical lifespan of between 10-12 years. As a consequence, potentially affected patients should already be thinking about replacement. Given the age of the implants (and their accepted lifespan), any procedure will be undertaken in accordance with our normal terms.
PIP Update - Group Litigation Order
9th March 2012
Group Litigation Order (GLO) was made today in Cardiff with regards to PIP implants. The purpose of a GLO is to group together claims and claimants with common issues. Both Transform and our insurers were represented at the hearing today and supported the granting of the Order. Transform believes a GLO is a sensible way forward. It will avoid multiple proceedings in different courts and will ensure a consistent and fair approach is taken to resolving claims. Our hope is this will be a further stepping stone to promptly and fairly determining all matters.
We will publish updates on the outcome of these proceedings as they become apparent.
PIP Update - Department of Health Reported Statistics
24th February 2012
The Department of Health has published their second set of figures in relation to PIP cases being attended by the NHS.
Transform patients constitute 3.2% of the total NHS case load to date (112 out of 3512 referrals).
Since the PIP crisis began Transform has been extremely conscious of our duty of care to our patients. Transform continues to offer all Transform PIP patients the following:
- Free consultation
- Free scans (MRI and Ultrasound)
- Free removal (this is the same offering being made by the NHS)
- Removal and re-augmentation at no commercial gain (£2500) and free access to medical records.

Today the Department of Health announced that 2860 PIP patients had been referred to them from private clinics.
Since the PIP crisis began Transform has been extremely conscious of our duty of care to our patients.
In the interests of balanced and transparent reporting, it is important to have a context to the figures published by the DoH.
- Transform has 5,021 PIP patients
- Of those 2,152 are already booked in to or have seen a surgeon
- In the last 2 weeks we have booked and completed 131 free scans
- 309 patients have either had a PIP operation or are booked in to have one
For now, Transform continues to offer all Transform PIP patients the following:

PIP Implants - Latest News

Media commentary over the last 48 hours has focussed on two topics concerning private clinics:
1) The refusal to attend to conditions related to rupture (such as removal of lymph glands)
2) Encouraging or pressurising patients into undertaking additional breast procedures (such as uplift or reduction)
Both these points require clarification.
1) Treatment of related conditions
There are many factors to consider prior to lymph removal and every patient's individual circumstances differ. Where such cases arise both patient and surgeon will discuss the safest and clinically most appropriate course of action for the patient concerned. This may involve directing the patient to their GP / another healthcare specialist.
Patients affected by siliconomas or lymph nodules may or may not require clinical care beyond that which is available from a cosmetic surgery provider. Lymph gland removal is not a cosmetic procedure and it would be clinically irresponsible for any surgeon to undertake a procedure in an area where they believe it is not in the best interest of the patient to do so.
2) Pressurising of patients to undertake further procedures
In all but a small minority of cases Transform has not used PIP implants since 2005. With almost 10 years having elapsed since their original surgery, some patients have experienced changes in their weight and bodyshape.
During their PIP review consultation patients and surgeons will discuss the likely outcome of their PIP reaugmentation. It may be that to achieve the result desired additional cosmetic surgery such as breast uplift, reduction or nipple correction may be required. As these additional requirements are unrelated to the issues surrounding PIP there is an additional cost.
Any recommendations are made purely from a clinical standpoint and the subtext of the media commentary is misinformed and highly derogatory. We are extremely conscious of the sensitivities of our patients affected by PIP and are appalled by the suggestion we are trying to capitalise on their situation. We would never pressurise or encourage patients to undertake additional or unnecessary surgery.
We continue to offer free consultations, free scans for all our PIP concerned patients, free removal and removal and reaugmentation at £2500 (an amount at which there is no commercial gain for Transform).

Transform met with representatives of the Department of Health on the 13th February 2012 to discuss the ongoing PIP implant situation. Transform had been requesting a meeting with the Department of Health since early January, to highlight the issues experienced by our patients and lobby their case with Government. At the meeting Transform worked hard to emphasise the level of anxiety and deep concerns experienced by our patients and highlight to the department the importance of supporting those with PIP implants.
The Department of Health’s representatives undertook to feedback Transform's comments and highlighted that two enquiries are underway to establish how this situation arose, the role of the Medicines and Healthcare products Regulatory Agency (MHRA) in licensing these products for use in the UK and to consider future regulation. Transform will continue to offer free scans and consultations for all patients. We continue to offer removal and replacement of PIP implants at a level of investment for patients that ensures we make no profit from the procedure. Please be assured we are making every effort to lobby the Government on behalf of our patients. Transform operates to the highest clinical standards and continues to lead the way in industry best practice.

The Department of Health today announced two wide ranging reviews relating to PIP implants.
The first (due to be published in 2 months time), will cover the sequence of events, information available and actions taken since the Department of Health first became aware of the French PIP situation in March 2010. The second review will cover regulation in cosmetic surgery and standards of clinical governance. This review will be lead by NHS Medical Director, Professor Sir Bruce Keogh.

The DoH announcement comes at the same time we are able to confirm how we are attending to patients with PIP implants. We thank all patients who have been bearing with us during the last few weeks.
Moving forwards, we will be offering:
1) Free removal
For any patient who so wishes who had PIP implants at Transform since 1st January 2001, we will undertake free implant removal.
2) Free scan for all patients
For all patients with PIP implants since 2001, we are offering a free scan for their immediate peace of mind.
3) Removal and replacement
Having reviewed costs with our partners, for patients who wish to have their implants replaced (whether ruptured or not), we will undertake this procedure at a cost of £2500. There is no commercial gain for Transform in this price.
4) Patients with valid warranty
For those patients who had their operation post 2006, they may still be within their warranty period. For those patients, we will undertake free removal and replacement.
Note
- Free scan is available only to patients who arrange their scan appointment via their Patient Co-ordinator for a BMI Hospital.
- In instances where additional surgery is required (such as breast uplift or reduction), costs for this will be discussed at your consultation.
- The removal and replacement cost of £2500 will be undertaken with Allergan CUI implants. If a surgeon and patient both agree to other implants, an additional fee will apply.
- For patients who have already paid Transform for a PIP treatment (since the MHRA alert notice in March 2010), they will be eligible for a refund of the difference between the price they paid and the current price of £2,500. Those patients should contact their Patient Co-ordinator for further advice.

1) How do I find out if I have PIP implants?
Transform has not used PIP implants since 2005 with the exception of 108 patients. If you are a Transform patient and wish to establish if you have PIP implants, please call our PIP helpline on 08000 214 102.
2) Why did Transform not contact me sooner?
The MHRA and the Department of Health announced that PIP implants should no longer be used in March 2010. To date, the UK government has found no evidence to support routine removal of PIP implants.
3) What should I do if I have PIP implants?
If you are a Transform patient, you should first contact us on 08000 214 102. We will check your records and confirm if you have PIP implants. If you do and you would like to arrange a review appointment with a Transform surgeon, there will be no charge for this.
4) How quickly will I be able to see a surgeon
Patients who are concerned regarding PIP implants should seek advice. Our surgeons are available, free of charge, for any concerned patients to discuss the available options and offer appropriate counselling and guidance. We are giving PIP patients priority access to surgeons.
5) Will it cost me to access my medical records?
There is no charge to access your medical records. However, for reasons of patient confidentiality, requests must be made in writing to: Lindsay Mullins, Compliance Manager, Transform Medical Group, Manchester M22 4RZ.
6) How much will an operation to remove my PIP implants cost?
- The price for PIP implant removal only is £1850. You do still have the option to have your implants removed for free on the NHS. However, the NHS will not offer cosmetic patients re-augmentation.
- The price for PIP implant removal and replacement with Allergan CUI implants is a total cost of £2,650, which ensures the company makes no commercial gain from this situation. Do not add the removal only price of £1850 to this cost - the £2650 cost includes removal and replacement.
7) Why has this price fluctuated?
The price change is a reflection of us having negotiated with our suppliers, surgeons and clinical teams and securing the best possible prices.
8) Spire, Nuffield and other companies have offered to take implants out for free. Why is Transform charging?
Transform is committed to finding a workable solution for our patients. The Government’s stance is that there is no requirement for routine removal of PIP implants. We are charging a cost price at no commercial gain.
9) What implants will you replace with?
Since 2005 our implants have been supplied by US company Allergan. Allergan are the world's largest manufacturer of breast implants.
9) Why can't Transform just make a claim against their insurance company to fund the operations?
As neither the Department of Health nor the Government appointed regulator (the MHRA) has deemed the implants faulty, we are unable to involve insurance companies.
Finally:
Transform operates to the highest standards for clinical governance and adheres to best industry practice across the public and private sector. In respect of clinical standards, Transform is regulated by the Care Quality Commission in exactly the same way that the NHS is regulated. We have had exemplary reports from the CQC following random and unannounced inspections and these are available on the CQC website.
Our surgeons are recruited, appraised, audited, monitored and reviewed to a level that is at the very least commensurate to those employed in the public sector and we are proud to be associated with them. All are on the General Medical Council register. The GMC is the overriding body which sanctions, approves and allows surgeons to work in the UK. Our surgeons work solely in private cosmetic practice, which makes them some of the most experienced in the UK.

We must stress however that the The Medicines and Healthcare products Regulatory Agency (MHRA) has stated based on expert advice as it stands that “there is not enough evidence to recommend routine explantation of these (PIP) breast implants.”
We have engaged with the Department of Health at every stage of the review process regarding the PIP implant situation, offering constructive consultation and ensuring that they understood the level of anxiety expressed by our patients. We have also made all of our data available for review.
Rest assured that Transform operates to the highest standards for clinical governance and adheres to best industry practice across the public and private sector; this is not an issue of poor surgical performance or unsatisfactory aftercare.

Press reports over recent days suggesting Transform's PIP implant rupture rate of 7% is information that has been taken out of context and is misrepresentative.
In the interest of clarification and to alleviate undue anxiety amongst patients, Transform has not used PIP implants since 2005 with the exception of 108 patients out of the tens of thousands of procedures it has undertaken.The figure of 7% was based on seven cases out of the 108 patients. This is therefore far too small a sample to provide a robust statistical analysis.
Our advice to MHRA on sharing these findings was that more information was required. As a result (along with all other cosmetic surgery providers - and the NHS), we are complying with and are fully supportive of the government's request to furnish them with full statistics on PIP implants for the last 10 years. The government will consider those collective figures and our understanding is that the result of their collective findings will be made public in due course.

PIP Implants - updated guidance

Over the last 18 months the UK government’s health regulatory arm (The MHRA) has conducted extensive tests on PIP implants. They have also sought input from the UK’s Cancer Registry and this week have consulted other governmental health agencies both across Europe and internationally who have also carried out thorough tests and research.
Their unanimous conclusion remains there is no evidence to suggest PIP implants are linked with cancer.
However, there is evidence to suggest that PIP implants have above average potential for rupture. This evidence suggests that if rupture occurs, it is likely to take place in the first 12 months following implantation.
Despite the findings and research of the British and international health agencies, today, the French Government announced their recommendation that all concerned patients with PIP implants should have them explanted i.e. removed.
The basis for the French Government issuing this advice is based on the increased likelihood of the implants rupturing not a link to cancer.
Neither the UK Department of Health (or any other national body) support the position of the French government - the rupture rate in the UK is less than 1%, much less than it is in France.
A key time of concern is from 2005, when the manufacturing of the PIP implant changed – a layer of the outer shell was removed thus potentially increasing the rupture rate of the implants.

The Medicines and Healthcare Regulatory Agency (MHRA) is the British Government body responsible for the regulation of medicines and medical devices.
Following today’s press coverage regarding Poly Implant Prosthese (PIP) implants – the MHRA has reiterated its view that after extensive testing there is insufficient evidence to indicate an association with cancer and that there is nothing to support the routine removal of PIP implants.
If you are a Transform patient and are concerned, please be reassured that our usage of PIP implants since 2005 has been extremely limited.
The position of the MHRA is that current commentary from France is groundless and not rooted in fact but speculation. We will update our advice regularly based on information from the MHRA.

In addition to the French Governments investigations in May 2011, which stated that there was no evidence of any toxicity in the implant gel, the UK's MHRA worked with the Cancer Registry and concluded that there was no evidence of any assosiation with cancer.

The MHRA (The Medicines and Healthcare products Regulatory Agency) have issued the following statement regarding PIP implants:
The French medical device regulator, AFSSAPS, has released the findings of comprehensive testing carried out on Poly Implant Prosthese (PIP) silicone breast implants.
These results confirm the UK findings reported in September last year that there was no evidence of genotoxicity or chemical toxicity of the filler material inside the devices.
MHRA Clinical Director for Devices, Dr Susanne Ludgate, said: “We are extremely pleased that these further tests have again found no dangerous substances.
“Women in the UK with these implants can be reassured as a result of these findings.”

PIP Implants - updated guidance
We understand if you are concerned about PIP implants – especially in light of recent media coverage regarding PIP implants. We can now update you with the following guidance:
Between 2003-2005, Transform, alongside other UK providers, used PIP as one of our main implant suppliers. Before 2003 our use of PIP implants was limited and after 2005 these implants were used infrequently and only for implants over 525cc in size.
PIP implants came with the required CE approval standards as set by the European Union (EU) and were licensed for use by the Department of Health (UK Government).
The recent MHRA (Medicines and Healthcare Regulatory Agency), and French Government report issued September 2010, on PIP implants has concluded that the silicone used in these implants is not harmful.
Dr Susanne Ludgate, Clinical Director for the MHRA, stated: “there is no evidence for toxicity or adverse health effects arising from the filler.” Click here to read the full statement.

We currently use Allergan implants which are not PIP implants. PIP breast implants - UK test results
The MHRA (The Medicines and Healthcare products Regulatory Agency) have issued the following Press Release regaridng PIP implants.
The Medicines and Healthcare products Regulatory Agency (MHRA) has received encouraging results of UK testing on the silicone gel breast implants manufactured by the French company Poly Implant Prosthese (PIP).
The tests found no evidence of genotoxicity (potential for cancer) or chemical toxicity of the filler material in the implants.
The tests are not as extensive as those being carried out in France, but they have provided initial information as to whether there is a safety issue with the filler material.
MHRA Director of Devices Clinical, Dr Susanne Ludgate said, “It’s reassuring that our test results have shown no evidence of any associated risks with the filler material.
“We are however waiting for the results of the French tests which are more extensive and include mechanical testing of the implant shell because there maybe a suggestion of an increased rupture rate compared with other breast implants. We will update clinicians and women once these test results are available and provide further advice on patient management as necessary.
“If women have any concerns, they should speak to their implanting surgeon.
“Implanting surgeons should report any clinical and radiological problems associated with these implants to the MHRA Adverse Incident Centre
Breast implant 10 year warranty
